How to refer

You can refer into the service in many different ways:

Self-refer by calling: 01482 335451. We will take the necessary details, for example your address and date of birth and we will support you in making an appointment to speak to a member of the team.

Opening Times: Our team is available from 8am – 6pm, Monday to Friday.

Online: You can go online to refer into the service by visiting https://iaptportal.co.uk/erew.html
You will be asked to complete some questionnaires on how you are feeling and provide some brief details about your problems.

Text: Text talk to 60163, this will send you a link to the website.

GP referral: You can speak to your GP who can make a referral into the service.

For professionals: To make a referral into the service please use the referral form that can be downloaded here. Please return the completed form to hnf-tr.abservice@nhs.net.

What will happen after the referral?

After your referral has been made and reviewed by the team…

Our trained staff will read through your referral to see if our service will be able to meet your needs. If we feel we can meet your needs we may contact you via telephone or by writing requesting you contact us back to arrange an assessment.

The majority of the assessments are done via the telephone and these can take approximately 45 minutes to an hour

At your appointment time the therapist will call you on your preferred number, you will go through some questionnaires for the service and then have the opportunity to explain the reasons for referring into the service. During the assessment the therapist will also ask about your safety and safety of others.

At the end of the assessment the therapist will discuss the options for treatment either within our service or signposting/ referring to other services to best meet your needs.

We can offer a translation service, please request this when you contact the service if this is required.
